NEW CLIENT FORMS
If you have scheduled a therapy or counseling appointment, please print the New Client Forms, complete the forms, and bring them to your first appointment.
If you prefer to meet by Teletherapy, please complete the Teletherapy Consent Form and email it to therpst@msn.com before your appointment.
INSURANCE AND FEES
Bonnie participates in most insurance panels with exception of Medicaid and Medicare. She accepts credit cards. The cost per session is $125.00
You are responsible for obtaining information about your insurance benefits and for payment at the end of the session. We submit claims to your insurance company but expect you to provide information about your deductible, co-insurance, or copay. Refer to the behavioral health phone number on your insurance card for information.
